Grinders #1 & #2: The employer failed to ensure that the operator's hands were protected from being caught-between the clamp push-rod and rear of cylinder during loading operations. The operators were required to hold the rear of the cylinder for proper alignment of the cylinder into the drum ring while they pushed the clamp extension button, allowing the clamp push-rod to move forward and seat into the rear of the cylinder. |
