Employees were allowed to clean rollers and perform service and/or maintenance operations on Press machine SMB 4-2 without being provided with machine specific lockout/tagout procedures. Procedures were not developed, documented and utilized for the control of potentially hazardous energy. Employees cleaned Press machine SMB 4-2 rollers while they were energized. (b) Press machines SMB 4-3: On or about March 29, 2016 and at times prior thereto the employer exposed employees to hazardous energies (electrical and pneumatic) caught-in and struck-by hazards from press machine SMB 4-3. Employees were allowed to clean rollers and perform service and/or maintenance operations on Press machine SMB 4-3 without being provided with machine specific lockout/tagout procedures. Procedures were not developed, documented and utilized for the control of potentially hazardous energy. Employees cleaned Press machine SMB 4-3 rollers while they were energized.

The employees were allowed to perform maintenance and/or servicing on Tying machine 7-3 without isolating the machine from the energy sources to prevent accidental start-up of the machine or release of hazardous energy.
