Employees were allowed to perform maintenance and/or service on Shirring machine 9-1 without placing locks and tags on machinery for the purpose of controlling hazardous energy sources. (b) Shirring machine 9-9: On or about March 29, 2016 and at times prior thereto the employer exposed employees to caught-by, struck-by and amputation hazards from Shirring machine 9-9; Clams, Over-wrap fingers and shirring head. Employees were allowed to perform maintenance and/or service on Shirring machine 9-9 without placing locks and tags on machinery for the purpose of controlling hazardous energy sources. (c)Tying machine 7-3: On or about March 29, 2016 and at times prior thereto the employer exposed employees to amputation hazards from Tying Machine knife blade(Fly knife). Employees were allowed to perform maintenance and/or service on Tying machine 7-3 without placing locks and tags on machinery for the purpose of controlling hazardous energy sources. (d)Bubble Trap Machine 6-1: On or about March 29, 2016 and at times prior thereto the employer exposed employees to caught-by, struck-by and amputation hazards from Bubble Trap Machine 6-1. Employees were allowed to perform maintenance and/or service on Bubble trap machine 6-1 without placing locks and tags on machinery for the purpose of controlling hazardous energy sources. (e)Smoke Tower Machine 12-1: On or about March 29, 2016 and at times prior thereto the employer exposed employees to caught-by, struck-by and amputation hazards from Smoke Tower Machine 12-1. Employees were allowed to perform maintenance and/or service on smoke tower machine 12-1 without placing locks and tags on machinery for the purpose of controlling hazardous energy sources. |
