Citation text line 29 CFR 1910.333(b)(2)(iv)(B): Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that they were deenergized: a) Plant 2 Substation L2YK4902 SA; On or about May 2, 2016 the controlling employer GE Energy and Industrial Services, Inc. failed to ensure that employees of Universal Services, LLC. were protected from the hazard of electrical shock in that GE Energy and Industrial Services, Inc. as the controlling and qualified employer failed to test the electrical parts of equipment and verify they were deenergized prior to Universal Services, LLC. employees being exposed.
