Citation text line 29 CFR 1910.333(a)(1): The employer failed to ensure that live parts were deenergized before employees began to work on or near them: a) On or about 08/09/2016 - Transformer vault #1, an unqualified employee climbed up on a ladder to gain access to a live 13,800 volt transformer to remove a vent plug. The employee walked across the transformer, removed the vent plug, placed the plug and wrench on the transformer, and then turned and walked back to the ladder, got on the ladder where an arch flash occurred resulting in 1st, 2nd, and 3rd degree burns over approximately 17% of the body to the left arm, shoulder, chest, and back, lacerations to the head and back.
