29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) SOS Machines (Press ink rollers): On or about October 25, 2018 and at times prior there to, the employer did not ensure lockout tagout was used when the machine guarding was bypassed to clean ink rollers which exposed employees to injuries associated with being caught-in the ingoing nip point. b) SOS Machines (Bag Machine): On or about February 13, 2019 and at times prior thereto, the employer did not ensure lockout tagout was used when the machine guarding was bypassed to clean surfaces above the cylinder which exposed employees to injuries associated with being caught-in or struck-by moving machinery. Ampac Mobile Holdings, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(1), which was contained in OSHA inspection number 989617, citation number 1, item number 1a and was affirmed as a final order on September 12, 2014, with respect to a workplace located at 2050 Sixth Street, Mobile, Alabama. |
