29 CFR 1910.146(f): The entry permit that documents compliance with this section and authorizes entry did not identify the required information listed in (f)(1)-(f)(15): a) Lime Day Bin; on September 15, 2015 and times prior the employer exposed employees to chemical burns while changing lime filters in that confined space permits did not contain all necessary information including but not limited to: The name of the permit space to be entered. A list of the authorized entrants. A list of the personnel, by name, currently serving as attendants. The individual, by name, serving as entry supervisor. A list of the hazards of the permit space to be entered. The measures used to isolate the permit space and to eliminate or control permit space hazards before entry. A description of the acceptable entry conditions. The results of periodic tests to include when the tests were taken. A list of the rescue and emergency services that can be summoned and the means for summoning these services. Listing of communication procedures used by authorized entrants and attendants to maintain contact during the entry. Any other information whose inclusion is necessary, given the circumstances of the particular confined space, in order to ensure employee safety. |
