The Coastal Forest Products, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard [1910.147(c)(4)(ii)(B)], which was contained in OSHA inspection number 1094724, citation number 1, item number 1a and was affirmed as a final order on December 15, 2015, with respect to a workplace located at, Plywood Plant, 102 Main Street Chapman, AL 36015.

As an employee was working on the cylinder rod connection of the batwing, the tack weld broke allowing the batwing to fall onto both arms of the employee.

On June 30, 2012 an employee while unjamming the unloader while it was in operation placed his hand between a chain and sprocket resulting in a crush / amputation injury.
