The Coastal Forest Products, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard [1910.147(c)(4)(ii)(B)], which was contained in OSHA inspection number 1094724, citation number 1, item number 1a and was affirmed as a final order on December 15, 2015, with respect to a workplace located at, Plywood Plant, 102 Main Street Chapman, AL 36015. |

As an employee was working on the cylinder rod connection of the batwing, the tack weld broke allowing the batwing to fall onto both arms of the employee. |
