29 CFR 1910.146(c)(5)(ii)(C): Before an employee entered a space, the internal atmosphere was not tested with a calibrated direct-reading instrument for oxygen, flammable gases and vapors and potential toxic air contaminants: a) On or about February 21, 2019 and times prior: confined space: An employee was observed cleaning the ferment #2 tank from an A-frame ladder using a water hose to spray the inside of the tank when he stuck his face through the opened hatch into a permit-required confined space without taking any internal atmosphere readings for a visual inspection of the tank walls to ensure they had been properly cleaned. b) On or about February 21, 2019 and times prior: confined space: An employee cleaning the blender tank from an A-frame ladder using a water hose to spray the inside of the tank stuck his face through the opened hatch into a permit-required confined space without taking any internal atmosphere readings for a visual inspection of the tank walls to ensure they had been properly cleaned. c) On or about February 7, 2019 and times prior: confined space: An employee cleaning the four ferment tanks from an A-frame ladder using a water hose to spray the inside of the tank when he stuck his face through the opened hatch into a permit-required confined space without taking any internal atmosphere readings for a visual inspection of the tank walls to ensure they had been properly cleaned. |
