EE was taken to the doctor and was diagnosed with a left shoulder strain and released that day for light duty. EE returned for a check up on 1/25/17 and was told that it may be a torn rotator cuff. MRI to follow.

Narrative The trained worker was washing out the mixer. He placed his hand in the mixer contrary to his training to brace himself to wash the mixer. In the act of moving the water hose, the hose hit safety switch. This turned on the mixer & pulled his left hand into the mixer. He broke bones in is left hand.

Narrative The worker was loosening a nut at the slide gate on a hopper. The nut would not loosen at first, so the worker hit the wrench with is left hand to break the nut loose. When he hit the wrench a sharp pain went up his arm, which caused his lower left forearm tendon to become inflamed.
